Dictation is only as good as the audio it hears and the language pack it is using. A distant or noisy microphone, a language variant that does not match your accent, and a missing speech model each cause exactly this. Aggressive noise cancellation in meeting apps also chews up speech before it reaches dictation. The quickest thing to try first: fix the microphone first — Settings, System, Sound, Input. Pick the right microphone, click it and speak; the volume bar should reach two thirds. Set input volume near 100. Match the speech language to how you speak
Settings, Time and language, Speech, and set Speech language to your variant, for example English (United Kingdom) rather than English (United States). Tick Recognise non-native accents for this language. The wrong variant misreads vowels constantly.

Install the speech pack properly
Settings, Time and language, Language and region, click the three dots by your language, Language options, and make sure Speech recognition and Enhanced speech recognition are installed and not stuck downloading.

Train it on your voice
Search for Speech Recognition in Control Panel, open it, and run Train your computer to better understand you. Read the passages once. Also click Voice Recognition, Advanced speech options, and tick Review documents and mail to improve accuracy, which lets it learn your names and jargon.

Turn off noise suppression while dictating
In Settings, System, Sound, click your microphone and switch off audio enhancements. In Teams or Zoom set noise suppression to Low or Off. Heavy suppression clips the start of words, and dictation then guesses.

On a Mac, use the on-device model
System Settings, Keyboard, Dictation, choose your language and pick a variant that matches your accent, and download the language so it runs on the device. Under Microphone Source pick the headset rather than Automatic.
If none of that worked
If it is still poor with a headset, try Google Docs Voice Typing or Word's own Dictate button; they use different models and one usually suits your accent better than the built-in tool.
