Why does my external monitor show an active signal resolution higher than my desktop resolution?

Short answer

There are two different numbers here. The signal resolution is what the graphics card actually sends down the cable. The desktop resolution is the size of the image Windows draws before it is scaled up to fill that signal. When they differ, something in between is scaling: display scaling in the graphics driver, a game or app rendering lower, or virtual super resolution. The picture goes sharp again as soon as both match. The quickest thing to try first: set the desktop to the monitor's native resolution — Open Settings, System, Display, and choose the resolution marked Recommended, which should match the panel, for example 3840 by 2160 on a 4K monitor. If the two numbers now agree in the monitor menu, you are done. Most people are done in about 10 minutes.

    Set the desktop to the monitor's native resolution

    Open Settings, System, Display, and choose the resolution marked Recommended, which should match the panel, for example 3840 by 2160 on a 4K monitor. If the two numbers now agree in the monitor menu, you are done.

    Turn off graphics driver scaling

    In NVIDIA Control Panel open Adjust desktop size and position and set Perform scaling on to Display, then untick Override the scaling mode set by games. In AMD Software open Display and switch GPU Scaling off. In Intel Graphics Command Center open Display, General, and set Scaling to Maintain Display Scaling.

    Switch off super resolution features

    NVIDIA calls it DSR, AMD calls it Virtual Super Resolution. Both render a larger image than your monitor and shrink it, which makes the reported signal larger than your desktop. Turn them off in the driver control panel unless you deliberately want them.

    Check the cable and the port can carry the full signal

    For 4K at 60 hertz you need HDMI 2.0 or better, or DisplayPort 1.2 or better, with a cable rated for it. An older cable will still show a picture but only at a lower rate, which drives the monitor to scale. Try a different certified cable and the DisplayPort input if your monitor has one.

    Set scaling to one hundred percent to test

    In Settings, System, Display, Scale, choose 100 percent for a moment. If everything becomes sharp but tiny, the softness was Windows scaling text, not a signal problem. Put it back to 150 percent, which is normal on a 4K screen, and sign out and back in.

    Set the monitor to fill rather than stretch

    Use the buttons on the monitor to open its own menu and look for Aspect, Image Size or Scaling. Choose 1:1, Native or Full rather than Aspect or Auto so the panel stops resizing what it receives.

If none of that worked

If a single game reports a different resolution, that is normal and set inside the game. Match the game to your desktop resolution, or turn its upscaling feature off, and leave the rest of Windows alone.
